Recognize that holistic health care may include the use of alter- native modalities, such as coining and cupping, as well as use of medicinal herbs, sauna, massage, and acupuncture. As possible and appropriate—and if such modalities do not interact with nutri- tion protocols and do no harm—consider how they might be woven into nutrition counseling and guidance.
Be aware that some people may seek health advice from both traditional healers and herbalists, perhaps in combination with Western medical practices.
Family Demands and Dynamics Multigenerational households are common in Southeast Asian cultures, and younger generations may shoulder the responsibility of caring for their elders. Among Vietnamese families, care for older adults is generally the responsibility of the oldest son’s wife or unmarried family members.34
Multigenerational households frequently abide by a patriarchal structure in which the oldest male is the decision-maker. Maintain the hierarchical structure when communicating with individuals and their families to involve and educate family members, which can help ensure that counseling recommendations are supported.
First-generation immigrants tend to prioritize the health of the male figures in the household, as they were seen as the main bread- winners. To support the men and optimize their health, women commonly reserve more protein-rich foods and vegetables for them, as those are deemed high-quality foods.
Vietnamese communities will often care for their elders at home and consider it disrespectful to place a family member in a nursing home.43
Nutrition Counseling Assess traditional food practices as well as Westernized food practices that may result from acculturation. Encourage cultural practices that include a variety of vegetables, fish, lean meats, and poultry as well as other nutrient-rich foods. As appropriate, utilize country-specific nutrition resources in the corresponding language, including MyPlate and a tip sheet in Vietnamese (see Resources on pages 442 and 443), and the available food- based dietary guidelines for Vietnam.44

Become familiar with local stores that sell produce and other Asian food products. Suggest healthy alternatives for Southeast Asian foods that may not be available. Generate a shopping list together that is flexible enough for various meal preparations.
Recognize that many popular Southeast Asian meals are mixed with several ingredients and served communal style. Determining amounts consumed may be challenging; see Chapter 2 for tips.
Address portion amounts of rice and, if appropriate, encourage smaller portions or rice noodles, especially for those managing diabetes. Also encourage consumption of more nonstarchy vegetables and lean protein foods. Suggest brown rice in place of some white rice during the week as a way to increase fiber, if needed.
Emphasize lower-sodium flavor options such as herbs and other aromatics for mixed dishes. Relying on fish sauce, soy sauce, pickles, and some other condiments as well as MSG for flavor can make traditional Southeast Asian diets high in sodium. Address the sodium content of processed foods for those consuming a more Westernized diet.
Encourage consumption of nondairy calcium-rich foods, such as tofu or fortified soy products, to increase dietary intake of calcium.
Reinforce the use of traditional cooking methods such as grilling, steaming, and stir-frying. Point out that steaming and stir-frying are great ways to retain the naturally occurring nutrients in vegetables.
For cooking methods that require oil, encourage the use of oils with less saturated fat, such as canola, soybean, and olive oils. Palm oil and coconut oil (as well as coconut milk), often used in traditional Asian cuisine in making desserts, are high in saturated fat. Use coconut oil sparingly and primarily for flavor.45
Encourage fruit as a nourishing dessert. Traditional Southeast Asian sweets made with coconut milk and coconut oil can be high in saturated fat and calories; being mindful of portion size and frequency when consuming these desserts is important.
Offer reminders that individuals can continue eating foods of their culture while following nutrition guidance and receiving medical nutrition therapy. Discuss the concept of moderation and that food choices can be flexible. Be aware that individuals and families may mistake food plans in handouts as strict prescriptions for dietary changes.
