malnutrition are widespread and attributed to deficiencies in food availability, quality of diet, and nutritional knowledge; inadequate health and nutrition of mothers and children; and micronutrient deficiencies of iron, zinc, and vitamin A.10 Te low level of education given to most Pakistan women, poor economic conditions, and low level of using public health care services are closely tied to higher malnutrition in Pakistan.11


Among children age younger than 5 years, about


40% experienced stunting and 17.7% experienced wasting, while the number who are overweight has almost doubled over 7 years, increasing from 5% in 2011 to 9.5% in 2018.10 More than half of households (63.1%) in Pakistan are


food secure; however, over one-third (36.9%) remain food insecure. Only one of every three young children receives complementary food between 6 and 8 months of age. More children in urban areas receive adequate complementary foods than their peers in rural areas, but rates are critically low in both localities.10 Cereal grains are the main staple in the Pakistani diet, providing 62% of total energy in the diet.10


Health Concerns of Pakistanis in the United States


Data on health of the Pakistani diaspora are limited, so it becomes necessary to rely on related data that include other South Asian diaspora groups in the US to build a richer view of the population.8

(people originally from India, Pakistan, Bangladesh, Sri Lanka, Nepal, Bhutan, or the Maldives) have a fourfold to fivefold higher risk for type 2 diabetes in comparison to other Asian migrant groups. Dietary patterns are an import- ant independent modifiable risk factor.17


Te Pakistani Te diet is wheat


based, eaten mostly with lentils and legumes. Te consump- tion of fruits and vegetables, fish, and meat (although highly desired) is very low due to widespread poverty. Fruit and vegetable consumption depends on local seasonal availability. Fluctuations in the availability of these foods are likely to be one of the factors responsible for deficiencies in micro- nutrients such as zinc, iodine, and iron, which are frequently observed in Pakistan.12


Disease Prevalence in Pakistan Cardiovascular diseases, cancers, respiratory diseases, dia- betes, mental disorders, and injuries have become the major causes of morbidity and mortality in Pakistan.13


According


to the second National Diabetes Survey of Pakistan (NDSP) conducted from 2016 to 2017, overall, 26.3% of adults have diabetes; and one in every four adults is overweight or has obesity.14


Hypertension (HTN) affects 18% of all adults and


33% of adults older than age 45 years. Hypertension rates are highest among people who are overweight or those who have obesity.15 Communicable diseases are prevalent in Pakistan due to poor sanitation, unsafe drinking water, overcrowded cities, low health awareness, poor socioeconomic conditions, and poor vaccination coverage.16


immigrant group constitutes a high-risk subgroup of the population that needs proactive prevention, early diagnosis, and timely management of type 2 diabetes, as incidences of type 2 diabetes tend to occur at a lower BMI compared with other ethnic groups, which puts Pakistanis at higher risk.18,19 Other common diseases include hypertension, cerebrovas- cular accident, coronary heart disease, cirrhosis, hepatitis B and C, hyperthyroidism, goiter, depression and dementia, pneumonia, chronic obstructive pulmonary disease, osteopo- rosis, arthritis, and stroke.15,20


Traditional Health Beliefs and Practices Healthy living is part of religious practice. Although Western medicine is used and respected, many traditional health beliefs and practices from hakim (traditional healers) are embraced as well. Misconceptions about food and health are widely prevalent in Pakistan, regardless of economic or education level. Health care providers are often asked about perhaiz, or particular food restrictions, as a way to manage a health condition. Such advice is based on cultural beliefs and often on perceived benefits rather than on scientific evidence.21


Furthermore, some belief systems, such as those described next, restrict seasonal and nutritious food choices.


Hot or Cold Food and Baadi Food Many Pakistani people follow a cultural convention of labeling foods as either “hot” or “cold” and use this to explain medical conditions and discomforts that occur when con- suming food. Te hot-cold theory is not about the tempera- ture or the spicy-heat of a food; rather, it implies how a food affects the body. Tis labeling comes from a belief in humoral medicine, which is based on the notion that the body is composed of four humors (or distinct bodily fluids) that
