CHAPTER 11 | Apply Standards of Nutrition Care & Review Effectiveness of the Nutrition Care Plan
Nutrition-related questions to address during each consecutive care plan conference include but are not limited to:
• Client goals and wishes: > What is the client’s understanding of their goals?
> Is there an understanding by the team of the client’s advance directives as they pertain to nutrition?
• Current diet therapy:
> Is the client tolerating and accepting of the current diet texture? If not, why?
> Is the client consuming adequate fluids? How is this known?
> Is the client tolerating artificial nutrition, if provided? How is this known?
> Does the client wish to have snacks, supplements throughout the day?
> Does the client accept or reject any supplements ordered?
> Is the client able to eat independently? If not, what assist needs to be provided?

> Does the client have any documented food allergies or intolerances?
> Has the client experienced any adverse reactions to food allergies or intolerances? What was the outcome?
> Does the client have specific cultural or religious preferences or avoidances?
• Weight history:
> Has the client’s weight changed since the last conference? By how much? What interventions are in place, if any? Are those interventions accepted by the client?
> Is the client on diuretic medications which will affect significant weight changes?
• Nutrition education:
> What concerns does the client have that would best be addressed through nutrition education?
> Does the client want or need nutrition education? Was it provided? When? Was it effective?
