Coordination of care: The CDM, CFPP interacts with the OTR mainly regarding adaptive feeding devices and positioning of the client during mealtime. Adaptive feeding devices will assist the client to remain as independent as possible during mealtime. The OTR may also be involved with the SLP and the rest of the IDT to determine the appropriateness of both diet and liquid consistency. The CDM, CFPP will then need to relay the information to the foodservice staff for the provision of meals and nourishments.
Licensed Clinical Social Worker (LCSW) Roles and responsibilities: • Evaluates social and support needs.
• Assists clients and families with decision- making.
• Helps clients and families plan care upon discharge from a healthcare facility.
• Assists with applying for other healthcare services, such as home-delivered meals or home care.
• Identifies internal and community resources. • Provides counseling.
Coordination of care: The CDM, CFPP interacts with the LCSW regarding client and family wishes. The LCSW is the primary contact for the facility if the client or family wants to discuss any changes or issues to care. The LCSW and/ or the Activities Director (AD) may coordinate a visit home for the client over the holidays or arrange for additional meals to be prepared if the family is coming to visit and wishes to eat with the client. Advance directives are legal documents that include answers to questions a person might want others to know in the event the person cannot speak for themselves. The National Institute on Aging (n.d.) reports that one study showed 33 percent of people did not know what their loved ones wanted at a time of need. Advance directives may include the wishes of the person concerning feeding issues up to and including artificial nutrition and/or hydration. The LCSW will connect with the IDT and client to discuss desires of the client concerning code status, physician orders for life-sustaining treatment (POLST), medical orders for life- sustaining treatment (MOLST), living wills, and advance directives.
