CHAPTER 11 | Apply Standards of Nutrition Care & Review Effectiveness of the Nutrition Care Plan
• Review the nutrition care plan and document any changes to the care plan. Review progress made towards the client- centered goals of the nutrition care plan.
• Review goals with the RDN or NDTR and prioritize client- centered nutrition goals.
• Prepare for what will be said regarding the status of the client—a bulleted list of facts may be useful to guide what will be communicated.
• If the RDN/NDTR is not available to attend the care conference, it may be necessary to report on his/her behalf.
• Anticipate how the client and/or family might react or respond to the nutrition care plan and be prepared to answer questions.
• Convey the message to the IDT regarding the nutrition care plan.
• Report on referrals made to other IDT members.
• Report the progress made towards the goals of the nutrition care plan and update/set new goals as warranted.
2. Be actively involved during the care conference:
• Listen and receive information from the other IDT members at the care conference.
• Listen for “new” information that impacts nutrition care.
• Look for the key points and specific answers to questions regarding client care.
• Give feedback to acknowledge understanding. • Ask questions for clarification.
• Discuss issues and concerns revealed in the care conference and work with the IDT to problem-solve regarding the improvement of client care.
• Listen intently to the client and family until there is a mutual understanding of their concerns.
The care conference is an important time to discuss the client in depth in a roundtable fashion with all the IDT members present. This is the best time for the client and family to ask questions with all the team members present, and to discuss the wants and needs of the client to ensure appropriate care is given. There may be more coordination needed at the conclusion of the care conference based on the needs of the client. If that is the case, the CDM, CFPP will again need to communicate with the RDN or NDTR and other members of the IDT to develop a plan for intervention, implement client-centered goals and approaches, as well as develop a plan for appropriate evaluation and follow up.
Reflection is an important part of a care conference. It may be valuable to think about the nutrition information and care that needs to be communicated upon conclusion of the care conference.
