Hot debriefs


We have introduced a hot debrief procedure, which takes place when a pati ent with a pressure ulcer or skin damage is identi fi ed. A senior nurse and a ti ssue viability specialist go out to review the pati ent and check that the right equipment is being used, as well as providing advice and support to ward staff .


The results have been impressive, with a 70% reducti on in the number of grade three and grade four pressure ulcers experienced by pati ents across the Trust. Grade two ulcers have dropped by 30%, with an improvement in the standards of assessment, documentati on and provision of care also noted.


The concept of hot debriefs are also being introduced into our falls preventi on system.
