Appendix Triage Unit and Location
1. The following personnel will constitute the triage unit for this facility: a. Charge nurse or other medical personnel.
Treatment Room 1. The treatment room for incoming casualties will be located in ___________________.
Identification of Residents 1. Resident’s identification
2. The DON or a designated representative is responsible to work commensurate with their professional back- ground and normal day-to-day activities. However, emergency actions may require performance of duties not related to everyday routines.
Public Information
1. Information to the news media or the public concerning emergency activities at this facility will be released only by the administrator. This is necessary to prevent the release of misinformation about casualties, serious injury, and so on.
2. Representatives of the news media will be directed to the office.
Disaster Inquiry Services 1. Should a major disaster occur in the facility, inquiries would be received from relatives concerning the welfare of the residents. All inquiries of this nature must be referred to the administrator. 2. Next of kin of casualties will be directed to the DON.
Care of Disaster-Caused Fatalities 1. After a resident has been declared dead by a physician or the coroner, the administrator will be responsible for: a. Notifying next of kin. b. Making mortuary arrangements with mortuary preferred by next of kin. c. Supervising removal of the deceased. d. Keeping record of above actions.
Location of Main Controls for Utilities
1. All department heads, shift supervisors, potential shift supervisors, and maintenance personnel will know the location and operation of the main controls for shutting off the gas, electricity, water, and oil leading into this facility. In the event of a disaster affecting any or all of these utilities, you might be the only person on duty available to alleviate the danger. For location of all controls, see section titled “Utility Controls.”
Review and Updating of the Disaster Plan
As circumstances warrant, changes will be issued to all holders of the disaster plan. It is imperative that changes be entered upon receipt.
The administrator will annually review the entire plan and revise as necessary. Specific Procedures
The remainder of the disaster plan consists of separate sections. In each section is a list of actions and support- ing data that it is expected will need to be taken in each of the various situations.
