particularly through ONS, has been shown to signifi- cantly reduce readmission rates in various older adult populations. The RDN plays a key role in helping the institutions’ efforts to decrease readmission rates through effective nutrition screening, assessment, and interven- tion. Ultimately, hospitals, postacute care settings, and SNF and LTC facilities need to determine best practices to help care for these older adults during their inpatient stay and after they are discharged to help stop the “revolving door” of hospital readmissions.
