Targeted acti ons to eliminate pressure ulcers
Their programme of targeted acti ons and interventi ons has ensured that, not only have there been no cases of acquired pressure ulcers during the past 18 months, but the team have also succeeded in healing existi ng pressure ulcers that were acquired elsewhere.
All pati ents receive a thorough check for pressure damage within two hours of arriving on the ward. Staff use the Braden Scale to establish each pati ent’s risk of developing a pressure ulcer and produce a corresponding care plan, which may include encouraging the pati ent to stand for one minute per hour, or the use of specialist equipment such as matt resses.
Mark Badham, clinical matron, credits the success to staff getti ng behind the changes:
“We’ve had great buy-in from the nursing staff , who are really pati ent focused. They know the risks and work hard to make sure that we minimise them as much as possible.
“I’m always looking for ways to improve the ward. We learn from incidents and look at how we can do things diff erently, but we also look at those things we do well to understand how we can keep doing them.”
