Specialist dementi a care

Our dedicated dementi a team were named as fi nalists in the Specialised Services Redesign category at the Health Service Journal Awards 2017.

The expanded team off ers specialist leadership, advice and support to staff , pati ents and families at Andover War Memorial Hospital, Basingstoke and North Hampshire Hospital, and Royal Hampshire County Hospital.

The service is available 365 days a year, with the team able to assess pati ents and provide specialist advice and support sooner, something that is proven to improve long-term outcomes. This year the team extended their hours into the evening and weekend to support wards in 1:1.

Alison McGinnes, clinical matron for dementi a service and frailty, said: “Our team work excepti onally hard to ensure that pati ents with dementi a receive the right level of care in our hospitals. This has a real impact in helping to reduce their hospital stay.

“We are able to spend more ti me working with staff on the wards, giving them really practi cal help, and the feedback from carers and families has been great.”

Heather Scott is a dementi a clinical support worker:

“I have specialist training and can support staff on our wards by talking to a pati ent or their family to fi nd out what pati ents like, what makes them agitated and how to calm them down if they get upset. I put this informati on into a care plan that helps the nursing staff to provide more personalised care for the pati ent.”
