An additional member is anyone on the same membership as the Individual. An Unlimited Group Exercise Add-on can be added to an active fitness membership only. This add-on will allow you to take all classes on the Group Exercise schedule. All participants must be 14 years of age and older.

Reinstatement of your unlimited group exercise add-on within 6 months of cancelling the add-on will incur a $25 reinstatement fee.

Enrollment fees are for newmembers or if amembership has been inactive formore than 60 days. Enrollment fees are non-refundable and themaximumenrollment fee per household is $150 whilemembership remains active. If a newmember is adding on to an existingmembership, the enrollment fee will be based on age: $79 for ages 23 and up; $49 ages 15 - 22; and $49 ages 62 and older. Enrollment feesmust be paid in full at the time of sign-up.

PAYMENT PLAN OPTION

If you are choosing the payment plan option, at the time of purchase you will be required to pay fromthat date to the date of the firstmonthly billing.

• Individualmembersmust be aminimumof 15 years of age.

• 12- to 14-year-olds can become amember once they successfully complete Teen Fitness University (additional fees apply).

• Householdmembersmust all reside at the same address. • Household of three ormorememberships can only include twomembers ages 24 and older.

• Seniormembersmust be 62 years of age and older. • Senior couplememberships can include only two seniors ages 62 and older and bothmust reside in the same household.

• Studentmembersmust be 15 to 23 years of age and provide proof of full-time status.

• Military - an individual actively enlisted, who can provide proof, in a branch of the armed forces.

• Persons 18 years or youngermust provide proof of age with a birth certificate.

• Children 12 years of age or youngermust be in Kidzone Childcare, in a programor directly supervised by a parent or guardian while in the facility.
