ASSESSMENT OF SHOULDER INSTABILITY SHOULDER INSTABILITY – PART II

KEY FACTORS OF ASSESSMENT So how do we identify patients? We have already described the younger patient who starts off subluxing or dislocating the shoulder joint as a ‘party-trick’ but over time finds that this pattern becomes established and involuntary. According to Malone (3) in the Polar type III group approximately half will present with pain and a third with recurrent involuntary dis- location associated with everyday use. Patients are often unaware that they are subluxing with movement and think this is ‘normal’. Patients rarely report any sig- nificant trauma – often they describe an innocuous knock in the playground or catching their arm on a door jam. Often the shoulder relocates spontaneously after the original event. In those patients who suffer a first dislocation with minor trauma and attend casualty due to an inability to relocate the joint, there are clues which make you suspect muscle pat- terning. Often the joint will be very diffi- cult to relocate and may require surgical reduction with anaesthetic. The patient may report that the joint then redislocat- ed quickly after reduction. It is essential to elicit an accurate description of the precipitating event in these patients.

The group that are often more difficult to diagnose are the subgroups ie. III/I, III/II, I/III, II/III. In the sporting popu- lation we commonly see the patients with a clear history of structural damage eg. the first time dislocator who reports significant trauma where the arm was sub- ject to a significant abduction/external rotation resulting in dislocation. In an ideal world, particularly if they are under the age of 25 and wishing to return to sport, they will have a surgical repair of the capsulo-ligamentous lesion and then return to normal. Unfortunately some patients will develop a patterning compo- nent after the first dislocation and some may develop it after surgery.

In our experience with surgical manage- ment of elite rugby players and overhead athletes, there is a tendency for individuals

to work on weights regimes emphasising the power muscles, namely pectoralis major, latissimus dorsi, deltoid, ham- strings and quadriceps. Isolated testing of the cuff and core stabilisers demonstrate significant control deficits. In the presence of a traumatic episode and the factors previously described one can imagine a scenario where patterning becomes established. Pectoralis domi- nance is common in this patient group and rehabilitation programmes post-op should be designed to address this. Similarly therapists working with over- head athletes’ preventative programmes should specifically address this risk. In

the hypermobile athlete or athlete with capsular laxity we appreciate we already have a proprioceptive deficit and probably also compromise in core control. Therefore the implications of a minor episode or response to change in training emphasis can not be underestimated in their poten- tial to precipitate a patterning problem.

WHAT TO LOOK FOR Clinicians experienced in dealing with muscle patterning are able to identify this problem clinically with relative ease in 67% of patients (3). However this leaves 33% of patients remaining. EMG can be a useful tool in confirming a patterning
