Constipation (continued) Increase fluid intake as tolerated; encourage fruit juices, prune juice, cider, and hot beverages. (The following recipe is effective and may reduce laxative use: Mix 2 cups applesauce, 2 cups unprocessed bran, and 1 cup 100% prune juice. Consume 1 to 2 oz with the evening meal.)
Discontinue calcium and iron supplementation if used; limit cheese, rich desserts, and other foods if these are constipating.
Diarrhea
Consider omitting the following foods if they cause diarrhea: milk, ice cream, whole grain breads and cereals, nuts, beans, peas, greens, fruits with seeds and skins, fresh pineapple, raisins, cider, prune juice, raw vegetables, gas- forming vegetables, alcohol, caffeine-containing beverages, and foods containing sugar alcohols.
Encourage bananas, applesauce, peeled apple, tapioca, rice, peanut butter, refined grains, crackers, pasta, cream of wheat, oatmeal, and cooked vegetables.
Avoid liquids with meals and instead encourage drinking liquids an hour aſter a meal.
Relax before, during, and aſter a meal.
If dehydration is a problem, encourage high potassium foods.
Mental disorders
Avoid alcohol and caffeine-containing foods, such as coffee, tea, and chocolate, if they contribute to anxiety, sleep deprivation, or depression.
If drowsy or apathetic, consider assistance with meal intake.
If caregivers are required for meals, encourage them to prepare the individual’s favorite foods, usually in soſt form to be served with a spoon or bite sized so that they may self-feed. Caregivers can shut off or remove knobs from stoves, remove matches, and lock doors to cabinets or closets that contain poisons, alcohol, or medications. Put away electrical appliances, such as mixers, food processors, can openers, and waffle irons; unplug microwave ovens.
If individuals are agitated or confused and caregivers are required for meals, caution them about the dangers of handfeeding the individual. Assist with a spoon and do not allow the individual to handle feeding utensils, plates, and glass. Caregivers can remind individuals what time of day it is, what meal is served, and what foods are served. Remind the individual that the foods served are favorites. Make mealtimes enjoyable by reminiscing about pleasant events in their life. Consider the pros and cons of waking the person if asleep at mealtimes.
If individuals are stuporous or comatose, counsel the family that semistarvation and dehydration are not painful to the individuals. Explore the possible pros (eg, increased length of life) and cons (eg, aspiration and need for suctioning) of handfeeding and enteral and parenteral nutrition support if they request information.

Supporting the Older Adult Who Can and Wants to Eat
For individuals who want to eat and who can be helped to eat better, it is important to enable them to eat as normally as possible. If indicated, medications such as megestrol
acetate, dexamethasone, mirtazapine,
olanzapine, and cyproheptadine can be administered to improve appetite and mood.35
The pros and cons of
these medications must be weighed, as all may cause substantial side effects. If anorexia is attributable to
correctable causes and the older adult has a predicted life expectancy of
several months, the correctable
causes can be treated aggressively if desired. Likewise, treatment should be aggressive if the anorexia appears to be an isolated symptom and the suspected conse- quence is malnutrition that could compromise both the quality and quantity of the older adult’s remaining time. Suggestions for improving oral intake of individ- uals using food are provided in Boxes 21.3 and 21.4.
