The Trust gained the support of NHS Digital to enter the Global Digital Exemplar programme, and this will support work to digiti se records and develop processes across the Trust in the future.


Case Study


Marc Berry is a senior improvement practi ti oner and part of the transformati on team, which works on improvement across the Trust. He coaches teams to develop new ways of working that will improve pati ent care, using tools, guidance and support.


“The SAFER pathway is the main thing we have been focused on for the last year. It gives each pati ent a plan that they know about. Everyone takes ownership of the plan and it helps pati ents to leave hospital quicker and get back to where they want to be.


“The seasonal “acti on” events had an impact on the Trust’s performance but also helped us to illustrate the impact that SAFER can have. It helped people to realise that it is not about doing anything radically diff erent. It’s just about having a structure in place earlier and focusing on what a pati ent needs.


“Over the next year, we will conti nue to work with wards, helping them to develop SAFER in their own areas.


“We are creati ng opportuniti es for people from across the Trust to come and have their say about how SAFER is working. We can then help them to develop their processes and identi fy areas for improvement.


“We’re going to look at how we run ward rounds and working even more closely with our partners in social care to fi nd out how we fi ll gaps and get people home. Supporti ng people to develop in their own areas.”
