Ensure ti mely cancer care for all pati ents Maintain waits for electi ve (planned care) below 18 weeks

Achieve our quality prioriti es Develop frailty pathways Reducing bed moves Safer maternity care Improved pati ent experience Improved discharge processes

Train 80 accredited quality improvement (QI) practi ti oners and 20 QI coaches

“RemEd” is an initi ati ve where a multi -professional team address the complexity of issues currently challenging the Emergency Departments through a number of ongoing projects. Projects currently include “smarter triage” for pati ents arriving by ambulance and the development of specifi c pathways including those for pati ents with abdominal pain, chest pain, fractured hips and unwell children.

In our “Season Acti ons” the Trust off ers extended services to prevent pati ents being admitt ed or help those who were well enough to leave hospital to do so as soon as possible. Led by the Trust, with support from Hampshire County Council, providers and clinical commissioners, the initi ati ves aim to reduce the number of people who need to be admitt ed to hospital and increase the number of pati ents discharged.
