The importance of good dental hygiene

From the very first ancient civilisations we can tell that human beings made attempts to clean their teeth. At the paleontological site of Atapuerca (Burgos) remains of teeth 350,000 years old were found, with traces of wear produced by the friction from instruments used to clean them.

Also, in the civilisation as old as that of the Sumerians, laboriously decorated instruments were shaped into the forms of toothpick and used for hygiene to pick between the teeth.

Of course back then they did not know about the existence of the bacteria which are deposited on the teeth or about the formation of plaque (and the need to brush to remove it). They were making the rudimentary motions to clean their teeth for two reasons: Firstly to alleviate the discomfort when food remains get between the teeth, and secondly for basic hygiene and cosmetic reasons.

These days it is proven that there is a direct relationship between bacterial plaque and the decay and diseases of the gums (gingivitis and periodontitis). It is therefore essential that teeth are kept clean to prevent the occurrence of these diseases.

There are various methods of brushing that may be suitable. For example, the Bass technique, which consists of placing the bristles of the toothbrush at a 45° angle on the edge of the gums and making circular movements to dislodge the plaque. It is important to brush all the faces of the teeth.

The use of dental floss is important for the efficient removal of plaque from between the teeth, and should be used daily. It is in the gum between the teeth where most often inflammation and disease start if they are not kept clean.

If it is the case that there is quite a lot of space between the teeth, then an interproximal brush should be used. These come in various sizes, and it is important to check that the right thickness of brush used is suitable for each gap.

For the correct cleaning of a fixed prosthesis (such as a bridge) a special thread can be used which allows the removal of plaque and the remains of food from underneath the bridge.

The gums surrounding dental implants can potentially become infected by plaque in just the same way that the gum surrounding a natural tooth can be. Cleaning, therefore, is just as important. The use of mouth sprinklers which throw out a jet of water under pressure is recommended.
