Online Registration Online registration begins December 1. To register log on to
LisleParkDistrict.org. Instructions for a successful registration are also noted on the website. Please note: you will be charged a processing fee for each completed online transaction.
How to register online:
1. Go to
www.lisleparkdistrict.org and click the “Online Brochure” button to access online registration information.
2. Register your household (by entering your login and pin if you have registered before). If you have not registered before you must come in with residency
verfication/ID and a current utility bill. 3. Read the step-by-step Online Registration Directions 4. Select the appropriate program category for your class. 5. Choose your class and proceed to checkout.

Mail-in Registration Early Registration for Winter/Spring 2012 programs will be accepted by mail at the
Lisle Park District Recreation Center, 1925 Ohio Street beginning December 1.
• NO CONFIRMATION POSTCARDS WILL BE MAILED. You may assume your enrollment is verified if you do not hear from the Park District. We will contact you ONLY if we cannot accommodate your request or the class does not meet minimum enrollment.
• Use the registration form found in this brochure. Registration forms that are not filled out completely will be returned without processing. Make sure that the waiver form is signed. Only checks, money orders, or credit card numbers are accepted. DO NOT MAIL CASH. Please include your driver’s license number on your check. Call the Park District if you need to request additional registration forms.

Walk-in Registration Walk-in registration for Winter/Spring 2012 programs will be accepted by at the
Lisle Park District Recreation Center, 1925 Ohio Street, and at the Senior Center, east entrance of the Recreation Center, beginning December 1 during regular office hours. VISA/MasterCard/Discover/Amex credit cards are accepted at both locations. Please bring proof of residency, such as a current utility bill, for walk-in registration.
By Phone You may register for programs by calling 630-964-3410 ext. 0 at the Recreation
Center or 630-969-0992 at the Senior Center during regular office hours. Telephone registration is available with the use of VISA/MasterCard/Discover/Amex credit cards only. We hope you will take advantage of this service, as we are trying to make your registration experience with the Lisle Park District as convenient as possible!
By Fax You may FAX in your registration, according to the same dates and procedures listed
above for mail-in and drop off registration. Fax registrations will only be accepted with the use of a VISA/MasterCard/Discover credit card. Fax to: 630-964-7448 at the Recreation Center or 630-969-5296 at the Senior Center.
After Hours Drop Box For your convenience, there is an after hours registration Drop Box located at the Com-
munity Center, 1825 Short St. Registration may be dropped off at this location after regular business hours while the building is still open for programming. Nonresident Registration Registration for nonresidents will be ac- cepted starting December 15 according to the same procedures listed for residents. Fees listed in this brochure are for Lisle Park District residents. Nonresident fees are 50% more than the listed fee.
Registration Policies 1. The Park District reserves the right to alter the schedule, cancel programs, or add programs when enrollments warrant. See page 54 for the Notice of Possible changes in Brochure Listings. 2. Absolutely no refunds will be made once a program has begun unless the Park District has altered or cancelled a program or if the “Satisfaction Guaranteed” policy is implemented (see page 54). 3. Programs that do not reach the mini- mum enrollment numbers will be can- celled. Cancellation of programs is made three days prior to the program starting date. In the event that the Park District cancels a program, the participant will re- ceive a full refund. If a program is can- celled in mid-session, a prorated refund will be issued. 4. There will be a $25 service charge as- sessed for all checks returned for non-suf- ficient funds. 5. There will be a service fee of $5 to cover the cost of processing a refund should an individual wish to drop out of a program. The Park District must be notified of cancellation no later than 24 hours prior to the start of a program. Refund checks are processed and mailed once a week on Fridays. 6. The Park District assumes no liability for personal injuries or for loss of personal property while attending Lisle Park District activities. 7. If any person wishing to participate in a Lisle Park District program or activity has special needs that must be accommodated to enable participation, that person (or par- ent/guardian) should notify the Park Dis- trict. The Lisle Park District will do what is reasonably possible to satisfy those needs.
