Peri-Implantitis and Laser Therapy: Shining a Light on Decision Making Donald S. Clem, III, DDS


Dental implants have arguably been one of the greatest advancements in dentistry. In the early 80’s clinicians were frequently told that these medical devices were essentially impervious to inflammatory bone loss, were lifetime solutions to edentulism and required little in the way of professional maintenance. As we have experienced in the ensuing decades, however, complications associated with progressive bone loss have been a challenge to the clinician putting patients at risk. Literally hundreds of dental implant manufacturers and a growing list of regenerative materials have complicated decision making in the face of bone and tissue loss.


Unfortunately for many clinicians this is a growing albeit perplexing part of current implant management. While a number of regenerative and resective protocols have been published, few have demonstrated a significant evidenced based approach with long term follow up. As multiple laser devices have been put into use, questions remain regarding their role in managing inflammatory bone loss around implants. This presentation will review the current evidence regarding peri-implantitis treatment and the possible role of Er:YAG lasers in managing these conditions. A suggested protocol with consecutive case series follow up and analysis will also be reviewed in detail.


The LAPIP Protocol: A Predictable Cost-Effective Peri-Implantitis Treatment

An increase in the number of ailing and failing implants is being reported worldwide. Peri- implantitis is becoming a growing concern in the dental community with estimates of 12% to 40% of implants placed being affected. Present treatment modalities, many which are based on periodontal surgical procedures, include surgical resection, regenerative therapies, and removal of the affected fixtures can be traumatic, technique sensitive and costly. A unique protocol, LAPIP, will be discussed that has been shown to be a more patient-friendly, cost- effective and a less traumatic way to treat patients with affected implants.
