Bespoke dermatology care

The benefits of personalised care in dermatology Skin concerns are ubiquitous, and for many conditions, including skin cancer, the number of people affected continues to increase year on year. Coupled with the expan- sion of medical treatment options to treat skin disorders and improve skin health, the importance of personal- ised care is more important than ever. Dr Bhargava discusses how this is delivered within his expert areas.

Skin Cancer “Basal cell carcinoma is the most common type of skin cancer in the UK.” While a standardised treatment approach is often applied, many surgical and

non-surgical treat-

ment options exist. “A treatment plan optimally suited to the needs of an individual can be formed by considering the microscopic features of a patient’s tumour and the loca- tion of the tumour as it relates to their unique anatomy. Tis is inte- grated with factors affecting their general health and their preference for a particular treatment type.” Dr Bhargava is in a small subset of dermatologists who regularly provide all the therapeutic options, including gold standard surgical care in the form of Mohs micrographic surgery. Tis treatment provides the highest cure rates for basal cell carcimomas, while preserving normal skin, which is important for tumours situated on the head and neck. Another example is the assess-

ment of a patients’ moles, for which he routinely uses a dermatoscope to enhance the accuracy of assessment. “Individuals often have a personal- ised pattern, like a fingerprint, that is common between their moles. Evaluating a suspicious mole can be enhanced by understanding how a single mole fits with this pattern.”

Hair loss and alopecia “I don’t treat hair loss; I treat a person with hair loss.” Treating hair loss

requires an assessment of the whole person, psychologically, physically and mentally. “It’s critical to acknowledge the psychological impact that hair loss may cause. Treatment often requires a long-term commitment, and it’s vital that individuals understand their condition and their treatment choices to make decisions that are right for them. Explaining their evidence-based choices in relation to what’s important to them is central in this’.

Cosmetic dermatology Cosmetic treatments are now widely available and are often delivered by protocols or numbers using injectables. “Tis is the antithesis of personalised care, homogenising the appearance of unique individuals. Personalisation is delivered by understanding and preserving

an individual’s unique

features and in meeting their personal treatment goals. Utilising lasers and devices, injectables, medications, skincare and surgery allows for the delivery of bespoke treatments. Tese can be used for a diverse range of conditions that include removal of skin growths, treatment of facial redness or pigmentation, acne scarring, wrinkle reduction, volume replacement and skin enhancement. By combining an understanding of the physiology of the skin, surgical anatomy, and a scientific knowledge of
