their medication and this vital link may prevent inadvertent errors and re-admission.
Although based at Biggart Hospital, the technicians are actually located within the HSCP Integrated Care and Enablement team or social work team, and have access to both health and social care IT systems from a single point, which has streamlined the process of gathering relevant patient information, and facilitates a joint approach to patient care after hospital discharge or in some cases preventing a hospital admission. The success of the project resulted in mainstream funding from September 2017, with resource included for additional CAP Technicians in East and South Ayrshire HSCPs.
GP Referral. Mr C, a 93-year-old gentleman who lives alone with no family was referred to social work by his concerned GP requesting carer support with medication, as his health had deteriorated in the last six months and he suspected non compliance. The CAP technician
contacted Mr C who admitted he often forgot to take his medication and how to re-order repeat supplies.
The CAP technician found out of date medication which she returned to the community pharmacy for destruction. CAP technician demonstrated various compliance aides, and Mr C felt that a blister pack, with a Pil Bob, was the best option because he had dexterity problems. Mr C lives in supported accommodation so a medication prompt was initiated through the check phone call. Follow-up visit by the CAP technician and a week later by social work colleague confirmed that Mr F was now managing his medication and his health was improving, and carer visits were avoided.
Little wonder then that the judges highlighted that the team displayed ‘great integration with health and social care’ and that this was a ‘new and exciting development of technicians using all their skills and providing convenient patient care’.
