inclusion in this test of change: prescription requests; medication queries; medication reviews (high risk medicines, polypharmacy); clinical treatment reviews (asthma)
The project ran one day per week for three months, from April to June 2019. Locum cover ensured protected time to undertake the activities, record interventions and results. During this time, Jane completed 48 pharmacotherapy interventions, with only 16 referred back to the GP.
The project objectives were achieved. Elements of the pharmacotherapy service could be delivered through CP, the clinical role of the CP was optimised and patient care was improved. The patient group had an increased awareness of the role of the community pharmacist in medicines management, support in managing stable long-term conditions, and ease of access to these services through ‘Pharmacy First’.
With remote EMIS access, the community pharmacist accessed patient records during consultations, and recorded interventions or
medicines prescribed immediately, which were fully visible to the GP team.
The Community Pharmacy Pharmacotherapy project demonstrates an innovative approach to collaborative working, ‘transforming the role of pharmacy across all areas of pharmacy practice, increase capacity, and offering the best person- centred care’ described in Achieving Excellence in Pharmaceutical Care.
Sharing the pharmacotherapy workload between primary care and community pharmacy, promotes collaborative working, improves clinical outcomes for patients, reduces workload for GPs, freeing up time to spend with patients with more complex care needs.
The judges pointed out that this initiative ‘highlights how a potential area of conflict and challenge can be resolved by collaboratively working towards a common goal, harnessing and utilising the strengths within each sector to achieve a harmonious outcome, realising an outcome greater than the contribution from each individual component’.
