Asset mapping helped share participants knowledge of local groups/organisations that could help support older people to remain active and involved in their community after the ten-week project. Asset mapping was also essential to continue the identification of frailty by the trained pharmacist so they could support and signpost older people to the appropriate local groups. Discussions were facilitated on what skills existed within the group and they how they could contribute to the content of the programme. This identified a chair- based exercise instructor who assisted with the physical activity task and an artist who shared their passion for mindful colouring.


Identifying skills within the group led to supporting cohort one participants to return to the project as peer mentors for cohort two. MeAAP were able to take a step back and allow the Health Centre Pharmacy Carrickfergus and the peer mentors run and support cohort two. each session ended with a free health check by the community pharmacist; one-to-one support and signposting, blood pressure check, height and


weight check, finger prick blood test for blood sugar and cholesterol levels. Participants completed a ‘start’ and ‘end’ questionnaire that focused on their awareness of health services, of their own health and of areas of improvement.

The Healthy steps to Ageing initiative recognised that frailty was a prominent health issue within Northern Ireland and the local community but did not take a top- down approach. Instead, it co-designed, produced and delivered the project through multi-disciplinary teams.


Co-facilitating Healthy steps to Ageing provided valuable insight and learning opportunities for all involved on how to facilitate guided discussions around frailty, which empowers and encourages older people. Insights gained will allow community pharmacists to inform, influence and support the older people they meet on a daily basis.
