and, as such, a range of health professionals should be involved in supporting patients with life- threatening diseases.
Pharmacists at hospices should be thoroughly involved in care, along with other medical staff and perform pharmaceutical services in order to deliver safe and efficient pharmacotherapy. They are a key part of the multidisciplinary team.
Patient safety is first and foremost, with pharmacy input to each patient care highly valued by the medical team, and is a reference point on a daily basis, with calculations on opioid rotations and drug compatibilities requiring advice frequently.
Pharmacy input to hospice policy matters such as Clinical Governance, Medicines Management and Audit of practice also helps improve patient care, with review of incidents and staff education based on relevant clinical topics.
The role has also allowed for initiation of pharmacy input to day hospice services at the hospice as well as
those based in community locations, such as Newtownards, Lisburn and Downpatrick.
In this instance David is able to offer specialist advice and information to patients attending these clinics, as well as help direct treatments options in collaboration with the medical/nursing team. Day hospice clinics based at the hospice also completed a BCPP project with David and sweeney’s Pharmacy in 2017, aimed at improving mental health and dealing with terminal illness, with input from mindfulness therapists and nutritional experts.
This new role has led to an upskilling of staff in sweeney’s Pharmacy, with David providing more information on the workings of the hospice and use of specialist medication, moving on from a stock provision only service, to a more rounded advice-based service with increased knowledge of palliative care. Having a pharmacist in the community with specialist palliative knowledge is novel and something that patients coming into their local chemist don’t normally have access to.
