on how to manage their own frailty in their homes and community.


The team was successful in gaining the funding to run the project in December 2018 but it was evident that, in order to identify skills and assets of older people living with frailty, training would be required for all concerned.


As a result, four community pharmacists from Health Centre Pharmacy Carrickfergus were subsequently trained by Carmel Darcy a Consultant Pharmacist (Older People) on how to apply the ‘Rockwood CFs’ to identify older people living with frailty noted as recoverable (3-5) and how to manage their drug treatment.


The training involved an introduction to the Rockwood scale of frailty and at what stages within the frailty cycle a community pharmacist would be best placed to make a difference. emphasis was placed on early intervention to help identify and prevent the older person moving to the next frailty level. The training looked at steps that a community


pharmacist could practically provide, which would help minimise risk of falls and injury such as exercise and diet.


The training also incorporated how a practical medication review could identify high-risk medications for frail older people, for example the use of sedatives and multiple anticholinergics which, when taken together, could increase cholinergic burden.


Once identified as ‘frail’, the older person could also be referred onto the pharmacy’s ‘healthy steps to ageing’ programme, which ran weekly over eight weeks and looked at various topics such as medicines, social isolation, depression, joint care and nutrition.


The project aimed to address the needs of 24-30 older people and their carers around frailty, isolation, low income, mental health and taking care of both themselves and others. It was also intended that it would enhance the partnership between community/voluntary sector, community pharmacists and local GP practices within the Carrickfergus area.
