Managing long term conditions such as strokes, Parkinson’s disease or respiratory conditions, it enables pre-operative pre-habilitation to strengthen before surgery, improving outcome and post- operative rehabilitation to guide you to recovery and full function.

Very often these can be delivered most conveniently in the home environment. Intermediate care plays an important role in preventing hospital


admissions and enabling early discharge from hospital. Working with occupational therapists, nurses and social workers,


physiotherapists assess strength, function, movement, coordination and balance. They carefully assess the risk of falls and offering solutions and signposting to other services, which may involve recommending exercises or equipment to help with moving about your home safely, or even accessing the outdoors.


Developing and maintaining personal independence has become a major goal for therapy


services and may involve liaising with other care staff and therapists.
