important to restore the natural balance between the three areas of the face, to contrast the processes brought about by ageing owing to a redefinition of the natural convexities. The ‘Gold Standard’ treatment for the upper third of the face is botulinum toxin. For the mid-face, it is advisable to use dermal fillers to restore volume loss. Good results can be obtained by using NASHA-based gels, injected by means of a microcannula or needle. NASHA gel has often been used for

treating the lower third of the face, yielding reasonably good results. However, the injection of NASHA gel simply fills the newly-formed concavities which ageing has created along the jawline. Accordingly, the final result is actually a mere illusion, in that no face-lift has been accomplished: on the contrary, the jawline has simply been lowered. This is why, in order to complete a 3D rejuvenation treatment, it is necessary to resort to Silhouette Soft Sutures.

A 3D rejuvenation Silhouette Soft Sutures are bidirectional bioresorbable sutures, with cones to obtain volume augmentation and skin tightening. The tailor-made procedure is effective in correcting skin ptosis and reducing the signs of ageing. The sutures are made of a clear resorbable material (polylactic acid) and eight resorbable cones, with a length of 30 cm. The eight resorbable cones are made of lactide glicolide 82 : 18; they are positioned on the suture and separated by knots. There are four cones on each side of the suture, free floating between tied knots each 0.5 cm apart. Each series of four cones faces the opposite direction and points towards the end of the suture, hence the term ‘bidirectional’.

The insertion of the sutures in the

adipose tissue occurs through an entry point in the middle of the suture pattern. The entry point enables the insertion of each half of the suture respecting the appropriate orientation of the cones. After the insertion of all the sutures in the treated area, a manual compression of the adipose tissue towards the centre of the cheek is performed to obtain the result.

and effective. It is simple, as it can be performed at the office and does not require any surgical incision or stitches; only one entry point has to be prepared by using a 19 G needle, with slow circular movements. Local anaesthesia is required only at the entry and exit points of the sutures, which are inserted under sterile conditions through the skin in the subcutaneous tissue. It is quick, requiring only 20–30 minutes to insert 1–3 sutures per side of the face. The procedure is safe, as no serious side-effects have been reported to-

date; only slight bruising usually lasting 1 week and oedema over the following 2 days. It is effective, as there are effective post-treatment results, no downtime and up to 18-month long-lasting results. The effects that can be obtained

consist in the retraction of the tissues and the repositioning of the cheeks, the increase of zygomatic projection

Conclusions An aged face shows visible changes caused by number of factors: laxity, reduction of volume, the effects of gravity, the redistribution of adipose tissue, and bone resorption. The practitioner’s effort is therefore aimed at restoring volume loss and redefining the shape of the face. A number of treatments are

available, according to the characteristics of the area treated. For the lower third of the face, Silhouette Soft Sutures are the best possible solution, in that they make it possible to obtain a real face-lift without incision and stitches. Furthermore, the immediately visible post-treatment results not only get better as time passes, but are also long-lasting.
