LOUISE COCHRANE NHS GREATER GLASGOW & CLYDE


Louise Cochrane has pioneered a technician-led domiciliary pharmacotherapy model supporting housebound patients in deprived communities. Through structured home visits, she delivers medication reviews, clinical observations, frailty scoring and blood sampling, resulting in 55 medication changes, 15 specialist referrals and a reduction in reported falls from eight to zero during evaluation. Her work improved quality-of-life measures and generated projected annual savings of £3,162. Louise’s proactive, preventative approach has enhanced equity of access, reduced unscheduled GP workload and strengthened medicines safety for some of the most vulnerable patients in Glasgow.


STEPHANIE ALLAN ROWLANDS PHARMACY SPRINGBURN


Stephanie Allan is recognised for her reliability, safeguarding vigilance and positive leadership within Rowlands Pharmacy. During a challenging staffing period, she supported a neighbouring branch to ensure continuity of compliance aid services and took responsibility for opening and securing the pharmacy. Her proactive safeguarding intervention when a vulnerable patient was uncontactable resulted in confirmation of their safety and personal thanks from the patient. Stephanie has also provided sustained support to vulnerable individuals through respectful follow-up and multi-disciplinary liaison. Her calm professionalism and team-first mindset strengthen patient trust and service resilience across her community.
