Dr Tania Ramos has demonstrated exceptional leadership in tackling health inequalities within an area of high socioeconomic deprivation. She established a pharmacist-led chronic kidney disease (CKD) review clinic and outreach medication reviews for housebound patients, improving blood pressure control, lipid profiles and medication optimisation. Her work has reduced kidney-damaging prescribing and strengthened early referral pathways. Securing research funding to formally evaluate and expand the model, Tania continues to embed pharmacist-led CKD care into routine practice. Her evidence-based, preventative approach has delivered measurable improvements in patient outcomes and equity of access.

Pharmacy Clinic Edinburgh has expanded access to safe, patient-centred prescribing through a modern, clinically led service model. The team provides same-day consultations for weight management, acute conditions and medication reviews, alongside structured private prescribing pathways supported by evidence-based PSDs. Particular impact has been seen in supporting international students and newcomers requiring continuity of prescription-only medicines. As an accredited Independent Prescriber training site, the clinic strengthens workforce capacity while maintaining high governance standards.
