are up to date; the staff were fully trained for the ride/activity and this was correctly recorded; whether the correct maintenance and inspection procedures were followed, and whether there have been any other similar incidents. You should be investigating all incidents, not just major accidents. All incidents, including near misses, can tell you a lot about how things actually are in reality. It is therefore important to have an environment where employees are engaged in Health & Safety and are encouraged to report all incidents. Once the investigation is completed, implement any necessary changes, ensure records, assessments and procedures are updated. You also need to effectively communicate the changes throughout the company and monitor to ensure changes/improvements are working.

Robert Kluth has over 35 years experience within the insurance industry, specialising in the leisure sector for the last 20 years. He is Chair of both BALPPA’s Health & Safety Committee and of the Amusement Device Safety Council, which oversees the ADIPS inspection scheme for amusement rides in the UK.
