Bat wing surgery


This is the colloquial term to define a procedure called “Brachial dermo-lipectomy” which consists of the removal of the loose skin and sometimes fat that forms at the top of the arms usually due to age, over- stretching or simply after a sudden weight loss.


Much the same as in other parts of the body, ageing is responsible for a reduction of the elastic fibres of the dermis. These are also affected by constant stretching forces which, combined over the years cause the fibres to give way giving the appearance of a loose fold of skin that goes from the arm pit to the elbow on the inner aspect of the arm. It is especially noticeable when wearing a bikini or short sleeved top.


‘Bat wings’ affect both men and women but are more evident on the later mainly because of clothing. They are much more common after the age of 50 for men and similarly, after menopause, for women.


The procedure is carried out under local anaesthesia


plus deep sedation on a day surgery basis. It takes about a couple of hours for both arms and afterwards the patient is sent home for recovery. Dressings need to be worn for 10 to 14 days after which normal activities can be resumed.


The operation involves the removal of skin together with fat (if any) along the inner aspect of the upper arm to the armpit. Sutures are made under skin so when healed, only a tiny white line should be visible.


The armpit is a very rich vascular area, also full of lymph glands and the brachial plexus. It is therefore important to have a good anatomical knowledge of it before attempting any procedure on it. When this is the case, little or no risk should be expected as all blood vessels, nerves and glands in the area are avoided.


The recovery time is of about one to two weeks. Initially the surgical wounds may turn pink, red or purple for a while. This is typical of the white race and more common in blond haired and blue eye patients but after some weeks this pink- red colour slowly fades resulting in a fine white line.
