Bichat cheek pad Treatment to slim the cheeks or face.


The buccal fat pad, known as “Bichat cheek pad” is present in all humans. It has its importance in early childhood as a reservoir of calories, to the point that its


absence is sometimes classed as mal-nourishment. Some people describe the buccal fat pad’s primary function in relation to chewing and suckling, especially in infants, thus giving children the usual round healthy cheeks. Later in life, the buccal fat pads still exist but become anatomically quite useless as fat develops in other areas.


In adults, the buccal fat pads are responsible for giving a round shape to the face. They are mainly located under the cheek bone, between the two muscles called buccinator and maseterus and they can be accessed from inside the mouth. The fat pads are surrounded by branches of the facial nerve and are in front of the parotideal gland and although they are easily removed, as with the face lift, anatomical knowledge is fundamental to avoid damage to these structures.


The removal of the buccal fat pads is usually carried out on a one day surgery basis combining sedation with local anaesthesia. A small incision of about one centimetre is made inside the mouth at the level of the 2nd molar tooth. The buccinator muscle branches are separated and the pad is accessed. Gentle pulling allows its total or partial removal and after that, one or two self dissolving stitches are placed on the inner part of the mouth. The whole procedure takes about one hour and the results will be seen after some days, once the swelling is gone.


The results: This operation can also be associated with liposuction of the neck, giving sometimes dramatic results. Ideally it is recommended to make the middle third of the face thinner (mainly in women) thus enhancing the cheek bones, especially in those patients needing more definition of the same. In men it can also define a more masculine profile as it reshapes the face to become a more angular one. Being a rather simple and safe procedure, I recommend it as a good substitute to cheek implants.
