VICTORIA INFIRMARY, GLASGOW Andrew Hart, Senior Clinical Pharmacist


The team of medical pharmacists at Victoria Infirmary have been working closely together over the last 18 months to develop and implement a new 'triage and referral' system within the medical wards of the hospital. The implementation of this system has allowed all members of the team to develop their skills into optimising the system. The team continually communicate with each other in order to raise any issues that have been identified and addressed as part of a team and are the first team in the country to develop this entirely new system in the medical directorate of the health board. This new system allows for pharmacy resources to be utilised to their maximum efficiency as the team can focus their efforts on those patients that are in need of pharmaceutical monitoring.


UNIVERSITY HOSPITAL, AYR Sarah Brown, Senior Pharmacist, Surgical Services/ Critical Care


A number of changes made at the hospital pharmacy department in University Hospital, Ayr has allowed the team here to provide the best service possible. UHA benefits from a Hospital Electronic Prescribing and Medicines Administration (HEPMA) system, and the team’s first move was to develop a report using information available from this system. The clinical pharmacy team also try to streamline the discharge process by identifying patients likely to go home early in the day and validating their discharge prescriptions at ward level. The clinical pharmacy team have made a significant redesign to the clinical service over the last 16 months. In achieving this, the team has worked well together, using technology and the specific skills of the team’s members.


LYNEBANK HOSPITAL, DUNFERMLINE


Natalie Bate, Communications Specialist Pharmacy The Pharmacy Services team at Lynebank Hospital work tirelessly to develop new and improved services within NHS Fife. New services including a Prescribed Sharps Disposal Service have only been possible with the endless hard work and determination of the team alongside greater communication and multidisciplinary working. The team at Pharmacy Services all have their own individual skills and expertise, and each brings their own unique viewpoint and proficiency to a project. Everyone understands the importance of ensuring everyone involved in a project is kept fully updated with progress, are involved from projects from the planning stage right through to launch and know who to ask for help when it is needed. The team work relentlessly, often far and beyond their job role to ensure the level of service and expertise they deliver on a daily basis is outstanding, and continue to show Fife Pharmacy as a leader in innovation.
