Your Rights & Responsibilities
PURPOSE: Te patient’s rights, responsibilities, and privacy practices have been established with the expectation that observation of these rights and responsibilities will contribute to patient care, greater patient satisfaction, and to assist patients in making decisions regarding their care by actively participating in their plan of care.
POLICY: It is the policy of Sierra Medical Center
to respect the patient’s rights to treatment and service subject to the hospital’s capability, mission, and applicable regulations Conditions of Participation 42 CFR 482.13. See also State Operations Manual, Appendix A, available at

1. Address patient’s involvement in all aspects of care including: 1. Informed consent. 2. When appropriate, the use of the family and/or surrogate decision makers to facilitate care decisions.
3. Resolution of conflict in care decisions. 4. Formulation of advance directives. 5. Decisions to withhold resuscitative services.
6. Decisions to forego and/or withdraw life-sustaining treatment.
7. Decisions relative to care at the end of the life.

Patient Responsibilities: To make patients aware of the responsi- bilities they have, which also extend to the patient’s guardian, conservator, surrogate decision-maker, or next of kin. Te care that you receive depends
partially on your participation and actions as a patient in the hospital environment. Terefore, in addition to your rights as a patient, you have certain responsibilities as well. As your healthcare providers, we believe that you, as our patient, and/or your significant other(s) have the responsibility to: 1. Provide accurate and complete informa- tion concerning your present complaints, past illnesses, hospitalizations, medications and other matters relating to your health.
2. Report perceived risks in your care and unexpected changes in your condition to the physician(s) and other healthcare providers.
3. Report any perceived or identified safety issues related to your care or the physical environment to your physician(s) and other healthcare providers.
4. Ask questions when you do not under- stand what you have been told about your care, or what you are expected to do regarding your care.
5. Follow your treatment plan established by your physician, including the instructions of nurses and other health care profes- sionals as they carry out your physician’s orders.
6. Participate in decisions regarding your medical care, including the planning and implementation of your plan of care.
7. Ensure the facility has a copy of your advance directive.
