By offering this understanding, community pharmacists gained a better grasp of the patient's journey, improving their ability to deliver care at a critical stage in recovery.


Initially, the teams faced challenges related to data protection and governance. However, passionate leadership overcame these hurdles, ensuring that sensitive patient information could be shared securely with community pharmacies.


Furthermore, test cases were created to refine the electronic documentation system, enabling safe and efficient service delivery.


One major challenge was the lack of clinical pharmacist availability for discharge validation, which led to a drop in patient numbers. However, strong communication and collaboration between the teams helped minimize these gaps.


Another challenge was the timing of discharge information. The original cut-off time was set for mid- afternoon, but due to patient rounds, some patients missed the opportunity to use the new discharge pathway.


The teams then agreed to extend the cut-off time to accommodate more patients, ensuring that no one was left behind.


The partnership brought substantial benefits to patients and the healthcare system. Faster discharges improved patient flow, allowing medically fit patients to return home sooner without waiting for their discharge medications. One patient's feedback highlighted this:


"I collected discharge medicines for my husband from the community pharmacy. I’m very happy with this service as he got out of the hospital much quicker than normal, and the medicines were ready for me to collect when I arrived at the pharmacy. The pharmacist also took time to explain all the medicines. A great service."

The service allowed community pharmacy teams to provide advice and care to patients during a critical stage in their recovery and rehabilitation.


Before this pathway, many community pharmacists were unaware of their patients' hospitalization and unable to offer targeted support.


Now, they can also make patients aware of other services like NHS Pharmacy First Scotland.


Overall, the collaboration between Community Pharmacy Ayrshire & Arran and NHS Ayrshire & Arran Trauma and Orthopaedics Teams has set a new benchmark for cross-sector healthcare partnerships.


Their commitment to patient care, professional development, and operational efficiency makes them worthy recipients of the 'Working in Partnership' award at the Scottish Pharmacist Awards.
