COMMUNITY PHARMACY AYRSHIRE & ARRAN AND NHS AYRSHIRE & ARRAN TRAUMA AND ORTHOPAEDICS TEAM


The 'Working in Partnership' category at the Scottish Pharmacist Awards celebrated excellence in collaboration between pharmacy teams and other healthcare providers. This year, the joint effort of Community Pharmacy Ayrshire & Arran and the NHS Ayrshire & Arran Trauma and Orthopaedics Teams was exemplary, earning them well-deserved recognition for their innovative work on the Community Pharmacy Discharge Service.


Launched in March 2023, the Community Pharmacy Discharge Service represented a new and innovative pathway for patients treated in trauma and orthopaedic wards across two university hospitals in Ayrshire & Arran. The service allowed medically fit patients to be discharged quickly and conveniently, with medications dispensed from a community pharmacy of their choice.

From the outset, representatives from Community Pharmacy Ayrshire & Arran and the NHS Ayrshire & Arran Community Pharmacy Team understood the need for strong collaboration. They worked closely together to develop a comprehensive service-level agreement and training program. The clinical team delivered training sessions to community pharmacists through networking


events and virtual sessions to ensure accessibility. Patient-centred care was at the heart of their collaboration, improving the patient experience through better access to medications and targeted advice from pharmacists.


The project helped streamline patient flow, reducing long waiting times and ensuring a seamless transition from hospital to community care. The
