act has developed a new ‘model of care’ for the community pharmacy network. this is a new concept of community pharmacies
working together to provide a population model of care. Pact has now become a strategic partner of the IMPactagewell social prescribing model, which aims to support older people in the Mid & East antrim area. Pact’s involvement in the project has ensured that community pharmacy is an intricate part of the hub model. this is achieved by the six Pact pharmacists, who represent the network of community pharmacies on the IMPactagewell hubs and sit alongside gPs, northern health & Social care trust staff and MEaaP Project officers to discuss each case.
the role of the six Pact pharmacists in the IMPactagewell project is to identify any health &
Social care Board community pharmacy service, which may be appropriate for the service user, such as Managing your Medicines, smoking cessation or the Minor ailments Scheme.
the pharmacists will work with the IMPact officer and local community pharmacies to ensure that, where appropriate, older people receive the required community pharmacy service. Pact has, with the support of partners, been successful in securing a grant from northern Pharmacists trust ltd to deliver a comprehensive evaluation of the community pharmacy aspect of the project. Pact is also now working with the Medicines optimisation Innovation centre to deliver a two-year independent evaluation of the community pharmacy element of the project.
