PAR-Q
The ‘physical activity readiness questionnaire’ (PAR-Q), originally developed by the Canadian Society for Exercise Physiology, provides a quick and easy method of identifying someone who may be put at risk by increasing their level of physical activity.
You can download an original free of charge from the Canadian Society for Exercise Physiology’s website.
Informed consent
As well as the PAR-Q, you should ask your client to complete an ‘informed consent’ form.
