Nutrition Assessment Client History
It is essential to assess each client’s relevant history, includ- ing the following factors (LD, HTN, DB, WM): • general client information, such as age, gender, lan- guage, literacy, role in family, and education; • medical and health history and medical

treatment,
such as goals of medical therapy and prescribed med- ications related to the medical condition for which MNT is being implemented and the appropriateness of weight management;
• social history, such as socioeconomic status, occupa- tion, social and medical support, cultural and religious beliefs, and housing situation;
• client or family medical history; • other medical treatments or surgical treatments; and • herbal supplement use, prescription and over-the- counter medication, and complementary and alterna- tive medicine.
Food- and Nutrition-Related History
Food and nutrition histories include food and beverage intake; energy and macronutrient intake; meal–snack pat- tern; bioactive substances (eg, alcohol intake, use of fish oil); nutrition and health knowledge, beliefs, and behaviors; food availability; PA; and readiness to change nutrition- related behaviors. The food and nutrition assessments recommended in the evidence-based nutrition practice guidelines for LDs, HTN, DB, and WM include, but are not limited to, the following: • food, beverage, and nutrient intake,1
including:
○ energy intake, serving sizes, meal–snack pattern, fat, types of fat, carbohydrate, fiber, and micronu- trient intake
