study by the CDC (2013–2014) shows an alarming increase in use of e-cigaretes among middle and high school students showing a tripling of use in a one-year period. As with all mind-altering substances there is the possibility of addiction serving as a precursor to the use of other more lethal substances. In other words marijuana may be considered a “gateway” drug. A “gateway” drug is any substance that poses an increased risk for both abuse/dependence on other substances. People do not use drugs looking for consequences, they use drugs for the effect. Te consequences arrive later based on dosing and dosage in the form of hangovers, nausea, vomiting, DUIs, jail time, institutions and death. Research shows which chemicals


within cannabis exhibit psychotropic or therapeutic medicinal value as an analgesic,


antispasmodic and so forth, specifically the phytocannabinoid cannabidiol (CBD). Cannabidiol is what the medically-ill desire, as it offers symptom relief. Currently progress is being made with a standardized plant extract derived from Cannabis sativa L., clone 202, which contains very high concentrations of CBD and minimal psychoactive compound, THC.7


Aside from naturally


occurring exogenous cannabinoids, humans are equipped with a naturally endogenous cannabinoid known as anandamide that corresponds to THC affinity for binding to CB1 receptors (psychoactive receptor).8 Following the discovery of anandamide another receptor, CB2, was identified and specific for binding cannabidiol. CB1 receptors with their atending psychoactive potential are predominately found in the central nervous system and widely


distributed throughout the gastrointestinal tract. CB2 receptors exhibit their activity in the peripheral nervous system, where they function in response to inflammation, immune defense, digestion and pain. Cannabidiol is the chemical of therapeutic


value and THC is the compound the recreational/social/abuser/user desires. Tis is where the tone, content and direction of the recreational and medicinal discussion may be headed. Will prescribers of medical marijuana one day be mandated to prescribe and direct their patients to the therapeutic cannabidiol enriched products over THC enriched products?
