Honoring Your Wishes


Everyone benefits when your wishes are spelled out and shared with those who could otherwise face very difficult choices at a very difficult time. It’s important to put your wishes in writing.


Advance Directives An Advance Directive is a written document that you complete in advance about how you want medical decisions made if a serious illness or injury leaves you unable to make decisions for yourself. An Advance Directive gives your family, caregivers and doctor specific information about how you want or do not want to be treated. Te two most common forms of Advance Directives are a healthcare power of attorney and a living will. A chaplain is always available to discuss these documents with you. Neither an attorney nor a notary public is required to complete these forms. If you have any questions about Advance Directives, please call the Center for Spiritual Care at extension 4744.


Healthcare Power Of Attorney Tis option allows the person you choose—called an agent—to make healthcare decisions for you if you cannot. Your agent must follow your stipulations about care and extraordinary measures to be taken or withheld on your behalf. Your doctor or other healthcare providers may not act as your agent. You can name a backup agent to act if the first one cannot or will not take action.


Living Will Tis option applies when you have a terminal illness and are unable to state your wishes. A living will lets your doctor know what, if any, life-sustaining procedures you agree to when death is imminent. If you have a living will and a healthcare power of attorney, the latter applies unless your agent is unavailable to make decisions for you.


Organ and Tissue Donation We participate in eye/ tissue/organ donations with the Illinois Eye Bank and Gift of Hope. If you or your family members are considering organ and/or tissue donation, please notify the nursing staff.
