ach is a 34-year-old architect who presents with an ankle sprain. He has a history of ankle sprains
and states that he never participated in any type of balance/proprioceptive rehabilitation program after the earlier sprains. While running recently, Zach stepped on an unlevel surface and suffered an acute right lateral ankle sprain. Clinically, Zach displays signs of hemorrhaging and swelling on the anterior, lateral, and posterior aspects of the right lateral malleolus. The area is tender to palpation, and Zach reports pain with weight-bearing and ankle movement. Using what you have just learned about the ankle and the foot, answer the following questions about Zach’s injury.
1. What was the most likely position of Zach’s ankle and foot at the time of injury that produced a lateral ankle sprain?
2. What ligaments are most likely affected in this type of injury?
3. If Zach has also strained muscle tendons in this injury, which ones might be involved? (Consider the ones in the same area as the involved ligaments that would be stretched at the end range of the position you identified in Question 1.)

1. What are the similarities and differences in the motion that occurs at the talocrural joint compared with the subtalar joint?
2. How does the bony configuration of the talocrural joint contribute to stability of the joint?

4. Which muscles can be involved when a person displays tenderness at the posterior and inferior aspect of the medial malleolus? Which muscles can be involved with tenderness at the posterior and inferior aspect of the lateral malleolus?
5. What is the action of the tibialis anterior from the point that the heel strikes the ground until the foot is flat on the ground during walking?
6. Which specific structures support the medial longitudinal arch? (Consider specific ligaments,
fasciae, and muscles.) How does the arch function during full weight-bearing and as the foot unloads in the gait cycle?
7. Demonstrate the position of the knee and ankle that most effectively stretches the gastrocnemius muscle. Demonstrate the position that most effectively stretches the soleus muscle.
8. Describe the components of pronation and supination at the talocrural, subtalar, and transverse tarsal joints. Include the position of the calcaneus in a weight-bearing position for supination and pronation.
