B6 FRED PLUM, 86 Neurologist advocated for right to die by Emma Brown

Fred Plum, 86, a neurologist who was one of the world’s fore- most authorities on coma and consciousness and a fierce ad- vocate for patients’ right to die, died June 11 at a hospice in Man- hattan. He had primary progres- sive aphasia, a form of dementia. Dr. Plum was the former head of neurology at Weill Cornell Medical College and New York- Presbyterian Hospital, and his re- search on comatose patients helped build a foundation for their diagnosis and treatment long before technologies emerged that allow doctors to measure brain activity. In the early 1970s, he helped coin the term “persistent vegeta- tive state” to describe comatose patients who appear to be awake and aware but are in fact un- conscious. Such cases have be- come more common as medical technology has helped critically injured patients survive. They raise difficult moral and legal questions about whether and how long an unresponsive person should be kept alive. Dr. Plum testified as an expert witness in several important court cases that helped establish precedents for allowing a pa- tient’s medical treatment to be discontinued. “He felt very strongly that peo- ple should die with dignity and control the end of their lives as much as possible and that lives should not be extended beyond the point where a person could feel his own humanity,” said his wife, Susan Butler Plum. Dr. Plum also spoke out about

the value of living wills, which al- low people to give instructions for their medical treatment should they become unconscious or unable to make decisions. In 1994, he cared for Richard M. Nixon when the former presi- dent had a stroke. Nixon died less than a week after the stroke, hav- ing made clear before he became unconscious that he did not want heroic lifesaving measures if he were brain-injured, Dr. Plum told the New York Times. “This was his last chance to ex- ercise moral leadership, and he did it,” Dr. Plum said. “Mr. Nixon made it apparent that persons of high order and dignity and posi- tion were quite able to sit back on the oars and make a decision that if they could not lead, if they could not contribute, and if they could not participate in society, then it was not worth living.” Fred Plum was born Jan. 10,

1924, in Atlantic City, where his father, a former world champion trapshooter, owned a chain of drugstores. He decided to study medicine after his sister died of polio. After graduating from Dart- mouth College in 1944, he re- ceived a medical degree from Cornell University in 1947 and worked during the Korean War at the U.S. Naval Hospital in Queens, N.Y. He then moved to Seattle, where he became chief of the neurology department at the

University of Washington. There, he and colleague Je-

rome Posner worked to figure out how best to examine, diagnose and treat the unconscious victims of head traumas and drug over- doses. In 1966, they published their findings in “The Diagnosis of Stupor and Coma,” a practical guide for doctors that has be- come a seminal text in the field and is in its fourth edition. In the early 1960s, he returned to Cornell to replace his mentor, Harold Wolff, as head neurolo- gist. A decade later, he and a Scot- tish neurosurgeon named Bryan Jennett coined the term “persis- tent vegetative state.” They also developed a standard way to measure and define a coma’s se- verity based on a patient’s move- ments and physical responses. The Glasgow Coma Scale is still used today. In 1975, Dr. Plum was the ex-

pert witness in the headline-mak- ing case of Karen Ann Quinlan, who had become unconscious af- ter mixing tranquilizers with al- coholic drinks. In one of the first cases testing a patient’s right to die, the hospital caring for Quin- lan refused her parents’ request to end her active medical care. A court ruled that she could be re- moved from her ventilator; she lived for nearly a decade before dying in 1985. Dr. Plum was also an expert witness in a 1990 Supreme Court case in which the parents of Nan- cy Cruzan, who had entered a persistent vegetative state after a
