Tummy Tuck It is interesting to consider that although in most cases losing weight will reduce the circumference of the tummy quite a lot, sometimes, especially after a certain age, the weight loss results in an excess of skin, causing a sort of apron that falls over the pubic area. This is a consequence of the distension of the elastic fibre of the skin, and the overstretching over the years by either gaining and losing weight or by childbirth.


When this happens it is not only a cosmetic problem but it is generally linked with skin infections in that area and cause of discomfort when wearing some clothes like Lycra underwear.


The only valid treatment for these cases is the removal of the redundant skin, together with the underlying fat and the tightening of the central abdominal muscles.


Years ago this would have required general anaesthesia and at least staying overnight in hospital, but now most of these cases can be operated under local anaesthesia and deep sedation on a day surgery basis.


There will be a first incision above the pubic hair extended to both sides. Skin together with fat will be lifted from the muscles up to the belly button or even to the lower edge of the rib cage. The two central vertical muscles (rectus abdominis) will then be tightened together rendering a sort of internal corset that will reduce the waist circumference a further couple of inches. The lifted tissue can then be stretched downwards to match the initial incision and then the excess removed together with the underlying fat. The navel is re-positioned, when needed, to its former location and the wound is stitched in 3 layers of subcutaneous self- absorbable stitches.


It is important to have relative rest for a few days, being able to do normal daily activities but not exercising nor carrying anything of weight for a couple of weeks. The scars are usually tiny and will be placed within the bikini line so they will always be concealed.


The total procedure takes about 2½ hours. Usually pain killers are advised and a normal follow up in about 2-3 weeks.
