Management of Long Term Conditions
The Chronic Medication Service is one of the core elements to the community pharmacy contract in Scotland. Pharmacists are now tasked with care planning and sometimes serial dispensing for patients putting them at the front of patient care. For the fi rst time patients with long term chronic conditions are now primarily managed by pharmacists on a national scale.
We want to recognise a pharmacist or pharmacy team who have embraced the chronic medication service. They will have delivered continued high quality care with evidence of improved outcomes for individuals. Are you or do you know someone who is excelling when delivering CMS?
General Awards Criteria:
• Enthusiasm and commitment to the delivery of high quality care with evidence of improved outcomes for individuals.
• An ability to overcome challenges in pursuit of goals
• How performance has been measured and benchmarked. How successful was the project? Considerations:
• Demonstration of excellent practice when delivering CMS to patients.
• Evidence of collaborative working with patients to help facilitate self- management of their condition.
• Examples of identifi ed care issues which have been addressed, improving patient outcomes.
Who Can Enter?
This category is open to all pharmacists working in either independent or chain pharmacies. Previous Management of Long-Term Conditions Award entrants, including winners, are also welcome to submit an entry. All entries will be judged by an independent panel of experts.

Community Pharmacy Team of the Year
This Award strives to recognise the pharmacy teams who have worked collaboratively on a project or in the general workings of the pharmacy and can demonstrate a signifi cant contribution to either patient care, business growth or the local community.
The winning team will be able to demonstrate their ability to deliver clear benefi ts through working together effectively, and effi ciently. The defi nition of a team in this context is two or more staff members from any one pharmacy sharing the same goals, in either their day-to-day work or on a particular project or task.
Entrants to this category must be able to show that they have used the principles of team working to achieve good practice and demonstrate how they have provided an improved service or performance as a result of multi- disciplinary team working.

• Exceptional quality above and beyond what is expected • An ability to overcome challenges in pursuit of goals
• How performance has been measured and benchmarked. How successful was the project? Considerations:
• How the team has demonstrated their ability to deliver clear benefi ts to patients; and/or staff members through working together effi ciently and effectively
• How the team has worked together to achieve its objectives over the past twelve months
• Projects that the team has successfully managed which demonstrate excellence in quality, innovation, productivity and prevention
• A clear display of the principles underpinning their success as a team Who Can Enter?
