Hills Community Care, a Hills Shire Council Community Service, started in 1991.
Hills Community Care conducts a variety of programs funded at Local, State and Federal levels to support people who are frail aged, or with dementia, and people with disabilities/mental health needs and/or their carers.
Located at the The Hills Shire Council Administration Building and Balcombe Heights Estate, 92 Seven Hills Road, Baulkham Hills.
Who is eligible to use our services?
Services are available to the frail aged, people with a disability, clients with dementia and carers residing within The Hills Shire, Blacktown City, Holroyd City, Auburn City and Parramatta City Local Government Areas.

SOCIAL DAY CARE PROGRAM
Phone: 9761 5707 or 9761 5706 When getting out independently to meet and make friends becomes difficult, Social Day Programs may be the answer.
Hills Community Care offers Cultural and Linguistic Diversity (CALD) specific programs for Korean, Filipino, Chinese & Maltese Communities.
What social day care offers: • Transport to and from programs • Planned activities
• Monthly outings • Companionship • Enjoyment • Morning tea/lunch/afternoon tea • Practical assistance as needed • All programs are wheelchair accessible

Specialises in assistance with particular personal transport needs from your door to your destination and home again. For information, see page 42.
DEMENTIA SERVICES Phone: 9761 5707
Provides flexible support to people with dementia and their carers. This assistance is aimed to enable people with Alzheimer’s disease or other dementia related illnesses to live in their own homes and participate in community activities whilst providing support and information for their carers. All services are provided by staff members that have an education and understanding of dementia-related issues.

MENTAL HEALTH RESPITE PROGRAM Phone: 9761 5724
Provides respite to carers of people with a mental illness, or people with an intellectual disability, living in the Council areas of Auburn City, The Hills Shire, Blacktown City, Holroyd City and Parramatta City. For more information, go to page 38.

EASY CARE GARDENING AND LAWN MOWING

Lawn mowing, up to approx. 900 sq. metres, and Easy Care Gardening service provide practical gardening help. Advice can also be provided on how to simplify gardens to make them more manageable. For details, go to page 54.
HOME MODIFICATION AND MAINTENANCE

WORKING CARER LIAISON OFFICER Phone: 9761 7607
The Working Carer Liaison Project (WCLP) offers assistance to carers who juggle paid employment with unpaid caring. The Working Carer Liaison Officer can provide you with up to 10 weeks of case co-ordination to help locate and establish appropriate support services for the person who is being cared for.

Volunteering is a commitment of time, effort and energy in helping others. The benefits of volunteering are many.
