LLOYDSPHARMACY, CUMNOCK HEALTH CENTRE, CUMNOCK Laura McKinnell and her team
have been involved in a new and exciting project integrating work between primary and secondary care to enhance the link between the community pharmacy sector and hospital setting.
The project is undertaken in a ward of a local community hospital, which is primarily GP-led, and was initiated with the aim of streamlining the ward discharge process. The partnership has continually developed from that point.
The new process put in place
for patient discharge is working well and Laura is taking on a more clinical role, visiting patients on the ward to discuss their understanding, compliance and technique with inhalers, drops, insulin etc. The position she has within the ward team and as a community pharmacist means she is well placed to flag issues to the relevant healthcare professional. She is also able to refer patients to the prescribing support pharmacists within the GP surgeries for polypharmacy reviews if need be.
This exceptional quality of care
which Laura and the team deliver by working closely with other healthcare professionals is above and beyond what is expected of them as a community pharmacy team. They are integrating and streamlining patient care from secondary care in the hospital to the primary care community setting, and their enthusiasm and commitment to this project will ensure that it is a success.
