a recent study has shown that almost two in five pharmacies do not keep adequate records of their dispensing errors. In the absence of a recording culture, it is impossible to identify trends or system weaknesses, or to share the learnings with your pharmacy team, improving the team’s overall performance.
Errors are inevitable. a standard part of medication safety training is the acknowledgment of human error and acceptance of our own limitations and that of our team. Pharmacists can utilise certain tools in order to assist the prevention of medication errors and reduce the risk of harm.
Pharmapod is an ideal system to record incidents and near misses. Using Pharmapod to record errors enables pharmacies to analyse errors and identify opportunities for quality improvement and system changes. additionally, the ability to generate safety reports helps pharmacists demonstrate compliance during regulatory inspections.
In a bid to address the issue with incident management and error reporting, a partnership has been developed between Ulster chemists’ association (Uca) and Pharmapod.
adam bartholomew, pharmacy manager and superintendent pharmacist of the healthcentre Pharmacy commented on the system saying that he ‘would highly recommend Pharmapod [to a colleague]’, adding that ‘the ability to integrate incident management with our SoPs in one place, whilst allowing us to analyse this information over time, ensures we are bring proactive about a difficult area that we are often reluctant to talk about as a profession’.
adam realised the benefits of Pharmapod early on and has been an advocate for the system ever since. When asked why he signed up, he said that the tool meant that they ‘could move from an antiquated, paper-based incident management system to a fully electronic approach’.
adam added that this ‘ensured that incidents could be quickly entered into the system, allowing for the prompt recall of information and the benefit of having an integrated statistical based analytical model’.
the superintendent recalled that the healthcentre Pharmacy previously incorporated a paper-based system, with all details being recorded on file, a method that many pharmacies still use. this method, however, presents an obvious analytical problem of linking safety-related trends and patterns. Pharmapod solves this problem by using its aggregated database to assess and present the data comprehensively: a system, which can be communicated across staff and pharmacies.
adam emphasised that Pharmapod also helps create a safety culture within his pharmacy through the ‘creation of a safety report where you can generate a document listing all incidents within a particular user-specified time frame’, adding that they ‘circulate this between all staff to ensure everyone is up to date with possible areas for concentration when it comes to patient contact, stock management and general dispensing’.
