The Double Chin


One very common subject which I receive many inquires about is the double chin. After the age of 50, many men and women are concerned about it as it is one of the most visible signs of ageing.


In some cases it is associated with localised fat in the area, in others it is just too much loose skin that may also be associated with wrinkles.


Because of this, the treatment may be different as the first could be benefit with a local neck liposuction (usually done together with some added liposuction to the cheeks) whereas in case of loose skin, the advised treatment is basically to tighten it and to tighten the muscles of the neck at the same time.


Liposuction of the neck is a safe procedure usually carried out under local anaesthesia plus sedation. It consists of 3 small holes of about a tenth of an inch in diameter. One under the chin and two behind both ear lobes through which a small cannula is used to aspirate the fatty tissue.


It normally doesn’t take more than 40-50 minutes and the patient is sent home on the same day. Dressings can be removed a day later and ordinary pain killers such as paracetamol are more than enough to deal with postoperative pain if there is any. The results can be visible after a day or two but as the liposuction also causes collagen fibres, together with connective tissue, to migrate to the area, there is a further re-absorption of fatty tissue and tightening of the skin to the muscles, so the improvement is constant and gradual for about 3 months.


In liposuction fat is removed together with fatty cells so once removed is not likely to come back, at least not to the treated area, therefore the results may be considered as permanent or stable.


When skin needs to be tightened a number of techniques have been suggested, all around a neck lift. Some suggest an incision under the chin and jaw in a moon shape form, eliminating both fat and loose skin. Usually this operation has very good results but it will leave a small scar under the jaw. Other techniques involve an incision behind both ears that either will leave no scar or will be totally hidden.


In many cases the right approach is achieved by a combination of techniques as it is normal the association of both problems, the excess of loose skin and an accumulation of local fat, so every case must be individualised and assessed in order to get the perfect approach.
