Partnerships in Care 9

Rehabilitation within PiC

In line with the philosophy of recovery based care, all rehabilitation services within PiC have a core value of working in partnership with patients, encouraging them to take increasing responsibility for their own mental health and ultimately decisions and goals in life. Patients are encouraged and supported in shared evaluation of their progress through the Recovery Star. The Recovery Star is used in the development of treatment goals and recovery plans as a part of the journey towards greater personal autonomy.

Patients benefit from dedicated social activity coordinators, operating outside the traditional 9-5 hours

For some patients, the transition to a service where they have a greater responsibility for their own lives is a challenge. The proximity of PiC rehabilitation facilities to a core hospital site, allows, where appropriate, a graded transition and a reassuring safety net in the event of a crisis.

Patients in our rehabilitation services are actively helped to understand the likelihood of relapse and work to develop appropriate personal management plans. It is important that patients are helped to see that relapse is not a failure nor should it lead necessarily to a re-admission to a secure hospital.
