Foreword


It gives me great pleasure to provide you with our 2011-2012 National Service Directory. Partnerships in Care (PiC) is a national organisation and this directory clearly describes the managed network of care pathways available for patients receiving care in any of our hospitals.


We are


committed to delivering a care pathway that is seamless and readily accessible


Our directory provides you with information about the services we offer, who is eligible for them and the types of treatment offered. The pathways described in this directory affirm our commitment to improving patient experience and choice whilst supporting a seamless, uninterrupted opportunity for recovery.


At a time of transition of the national commissioning architecture, there is an increasing responsibility on providers to ensure that services are efficient and supporting patients to achieve the best possible outcomes. This directory supports our clinical teams to actively plan future stages of treatment and intervene as early as possible if delays in the course of treatment emerge.


We firmly believe that it is the needs of the patient rather than the provider which determines the nature of care delivered. Our services will therefore continue to learn, evolve and improve through the introduction of initiatives such as this directory.
